The full route, consisting of 21 stages covering a distance of 3,479.9 kilometres (2,162.3 mi) and an elevation gain of over 47,000 metres (154,000 ft), was announced by RCS Sport on 24 February 2021. The race was bookended by two individual time trials in Turin and Milan, respectively.

Ganna „złapał gumę” i musiał przesiąść się na inny rower, co kosztowało go utratę kilkunastu sekund. Wydawało się, że z tego powodu pokona go Remi Cavagna, ale Francuz też miał pecha. Upadł 500 metrów przed metą i zajął drugie miejsce tuż przed Włochami Edoardo Affinim (o sekundę) i Matteo Sobrego (o dwie). Z czołówki klasyfikacji generalnej 30-kilometrową czasówkę najlepiej przejechał Portugalczyk Joao Almeida, który zajął w niej piąte miejsce i dzięki temu awansował na szóstą (z ósmej) lokatę w całym wyścigu z takim samym czasem jak piąty Daniel Martinez.
